Each student shall use the same “client” and type of disability that was used for the Functional Assessment exercise. This should represent one of the major diagnostic categories discussed in the text. This project will have two parts:

1. Completion of a referral letter. The referral letter will be from the Rehabilitation Counselor to the physician or other medical professional doing the evaluation.
◦Review the PowerPoints, Handouts, and Links on the medical evaluation and report writing contained in the Learning Modules for content and format information.
◦Use proper business letter format.
◦The referral should contain the following elements:
◦Introduction of the client, your role in the process, reason for the referral.
◦Referral questions which convey the specific information needed from the evaluator. (This is very important if the evaluation is to be worthwhile.)
◦Technical issues such as the requested timeline, where to send the report, your address, phone number, email address, payment procedures, etc.
◦Pay attention to proper spelling, grammar and punctuation.
2. Completion of the narrative report from the Medical Evaluator. Use the information in the Learning Modules as indicated above. The report needs to include:
•Observational information (objective information such as the client’s appearance, cooperativeness, lab results, etc.)
•Test data and data from other specialists
•Responses to your referral questions
•Conclusions about the client (subjective information)
•Recommendations regarding appropriate work settings, accommodations, and further medical or treatment needs.
